I would like to join and work with Kerala Taxi Driver's Organization. If I get the membership, I will follow the rules and regulations of the organization. I hereby declare that the details furnished above are true and correct to the best of my knowledge and belief and I undertake to inform you of any changes therein, immediately. In case any of the above information is found to be false or any kind of misbehavior found, KTDO officials can ban me from the organization without the refund of registration charges, monthly fee and donations.

ഞാൻ കെ ടി ഡി ഒ യിൽ മെമ്പർ ആവാൻ ആഗ്രഹിക്കുന്നു. മെമ്പർഷിപ് ലഭിക്കുന്ന പക്ഷം സംഘടനയിലെ നിയമങ്ങൾ ഞാൻ പാലിക്കുന്നതാണ്. മുകളിൽ കൊടുത്തിരിയ്ക്കുന്ന വിവരങ്ങൾ പൂർണ്ണമായും സത്യസന്ധമാണെന്നും മാറ്റങ്ങൾ ഉണ്ടാകുന്ന പക്ഷം ഉടൻ തന്നെ സംഘടനയിൽ അറിയിച്ചുകൊള്ളാമെന്നും ഉറപ്പു തരുന്നു. തന്നിരിയ്ക്കുന്ന വിവരങ്ങൾ തെറ്റാണെന്ന് അധികൃതരുടെ അന്വേഷണത്തിൽ ബോധ്യപ്പെടുന്ന സാഹചര്യത്തിൽ എൻ്റെ അംഗത്വം കെ ടി ഡി ഒ യ്ക്ക് റദ്ദ് ചെയ്യാവുന്നതാണ് എന്ന് അറിയിച്ചുകൊള്ളുന്നു. അംഗത്വം റദ്ദ് ചെയ്യുന്ന പക്ഷം എന്നിൽ നിന്നും ഈടാക്കിയ അംഗത്വ തുകയോ മാസവരിസംഖ്യയോ പ്രവർത്തന ഫണ്ടുകളോ തിരികെ നൽകുന്നതല്ല എന്ന കാര്യം ഞാൻ മനസിലാക്കുന്നു.
